Brookhaven officials have determined that the Jan. 1 propane gas leak in Shirley that caused the evacuation of 900 homes in the surrounding area was caused by "operator error."
Due to snow accumulations, a deliveryman mistakenly opened the valve unaware that there was nothing attached to the valve body except a plastic cap used to protect the threads from exposure to the elements. Once opened, the plastic screw cap was unable to withstand the pressure and began to leak gas.See the full content of this document
