Brookhaven Officials Say Operator Error Caused Gas Leak

Brookhaven officials have determined that the Jan. 1 propane gas leak in Shirley that caused the evacuation of 900 homes in the surrounding area was caused by "operator error."

Due to snow accumulations, a deliveryman mistakenly opened the valve unaware that there was nothing attached to the valve body except a plastic cap used to protect the threads from exposure to the elements. Once opened, the plastic screw cap was unable to withstand the pressure and began to leak gas.
